Fetch an OpenTopography item fresh and read a 4×4 window of a tile in bbox.

The catalog is static (no search API), so a probe of it is the loader’s own two steps: the item JSON must list a tile meeting bbox, and that COG must answer a ranged read. The item is fetched directly rather than from the loader’s cache, so a republished or withdrawn item shows up on the next run instead of after the cache expires.
